Therapeutic effect of newly-developed antifibrotic agent
pirfenidon compbined with home parenteral nutrition on
sclerosing encapsulated peritonitis

Pirfenidon is a new antifibrotic agent which inhibits the stimulatory
effect of TGF- and PDGF on fibroblats proliferation and on the
synthesis of intercellular matrix. Furthermore, this agent augments
collagenase synthesis.
This pilot study reports the effects
of Pirfenidon (40 mg/kg/day by mouth) in three CAPD patients with
sclerosing encapsulated peritonitis, a fatal complication of CAPD.
These patients were being treated with home total parenteral
nutrition (TPN). The outcome in these patients was compared with that
of a historical control group of four patients treated with
conventional supportive therapy including total parenteral nutrition
(TPN). Survival (16 Vs 6 months), body weight (+5 Kg ) and other
nutritional parameters were much better in patients treated with
Pirfenidon and TPN as compared with those treated with TPN and
conventional supportive therapy (Bruno Cianciaruso, M.D.,
University of Naples, Italy).
