ANNUAL COURSE DESCRIPTION

The 18th Annual Nephrology Postgraduate Course for the practicing nephrologist continues the tradition of providing in-depth information about selected areas of nephrology. This course has come to be recognized as a premier meeting which facilitates learning through close interaction amongst recognized experts and attendees in an informal setting. This year marks a transition from the traditional 4 half day sessions to include another half day with two optional workshops. These changes reflect the desire of a number of attendees to explore additional opportunities for hands on experience.

The first day of the course focuses on the Pre-ESRD patient. Several issues are addressed including a discussion of two controversial areas; the need and impact of early identification and referral of patients to the nephrologist and the use of structured protocols for management. While these topics are timely they also reflect the increasing recognition that this segment of the population deserves more systematic attention.

Thursday afternoon provides an opportunity to enroll in two optional workshops targeted to cover vascular access care and managed care issues. Both of these workshops are designed to allow a mix of didactic and hands-on experience. The faculty for both these workshops are recognized experts and we anticipate that the interactions will be highly rewarding.

Friday morning concentrates on management strategies in ESRD. Two areas of considerable controversy cover the ends of the spectrum of initiating and withdrawing dialysis. Both of these topics have significant impact on day to day management decisions the nephrologist must make. Friday afternoon=s session focuses on ICU nephrology, an emerging area requiring specific attention. Information about the changing nature of the critically ill patient is provided by two intensivists and the tricky balance between conservative management of pre-renal azotemia and initiating dialysis is covered. The vexing problem of renal involvement in hepatic failure is further defined. This session should provide an opportunity for nephrologists to hone their skills in the care of critically ill patients.

Saturday=s session presents a discussion of challenges the nephrologist is likely to face. Two of the topics cover problems faced by women, two others cover adrenal and lipid problems and the final one covers an emerging area of the long term consequences of transplantation. Each of the speakers will undoubtedly provide Apearls@ for handling these difficult situations. As always, we will strive to provide ample time for questions and to encourage vigorous discussion.

This year marks a transition in the running of this course from a seasoned, experienced clinician scientist to two relative novices in this area. Although it is difficult to fill Dr. Fanestil=s shoes, we hope to provide a informative and stimulating environment so that we can all learn form each other for the 18th time! We look forward to seeing you in San Diego in Februray.

1:30 - 3:30 WORKSHOP A: Vascular Access Care: The Multi-disciplinary Approach

This workshop is divided into three segments to cover several issues related to the care of chronic and temporary vascular access. There will be a initial lectures covering I) the issues of vascular access basics including choice of vascular access, principles of access flow, recirculation and access care and ii) radiological interventions to evaluate and maintain access patency. This will be followed by hands on demonstration of a) evaluation of chronic vascular access including the physical examination, static and dynamic pressure measurement, flow quantification and b) placement and care of temporary access (tunneled and non-tunneled). The final segment will provide a discussion on the economics of vascular access care including setting up and maintaining a database and will be followed by a questions and answer session.
