Total dose iron infusion: a time saving cost effective method of iron replacement

How best to give parenteral iron remains problematic in home dialysis patients, as it is impractical to have patients return to the clinic for multiple IV infusions. Whereas many PD patients, contrary to their counterparts on HD, can be maintained on oral iron, home patients also often require iron (although probably less often than center HD patients). Bower and Case developed the concept of giving a large amount of iron dextran at a single sitting. They wound up giving 700 mg diluted in 150 ml of saline, over 45 min to 2 hours to 110 patients, with only 3 mild reactions.

Comment: This mode of iron administration is not new; e.g., in the Dialysis Handbook it has been recommended to give 500 mg diluted in 250 ml of NS to PD patients when required. The safety of IV iron should be further increased with use of alternative agents to iron dextran (see 1996 ASN abstracts). (John T. Daugirdas, M.D., University of Illinois at Chicago)
