Reduction of infectious complications by temporary subcutaneous implantation of PD catheters

This paper, while not a controlled study, involved fairly large numbers of patients in a retrospective comparison of percutaneous implantation of PD catheters with immediate exteriorisation vs. use of the Moncrief technique (delayed exteriorisation). Each group comprised approximately 100 patients.

The peritonitis rate was 19% with the standard technique vs. 10% using delayed exteriorisation. The incidence of exit-site infection also was reduced. The authors conclude that delayed exteriorisation of PD catheters is a valuable method.

Comment: The question would be finally settled using a randomized trial. One other point to consider is the cost-benefit of routinely practicing detailed exteriorisation, as presented by Caruso et al at this same meeting. (John T. Daugirdas, M.D., University of Illinois at Chicago)
