The accuracy of polyclonal fluorescence polarization immunoassay- derived (P-FPIA) serum vancomycin levels in ESRD patients is unclear because of the presence of vancomycin degradation products which are also detected by this assay. To determine whether the P-FPIA overestimates vancomycin levels in PD patients, trough serum and dialysate levels were measured using P-FPIA and a monoclonal assay (M-FPIA) during treatment of peritonitis. No significant differences in values were obtained by the two assays, suggesting that degradation product accumulation does not occur in PD patients treated with vancomycin. (David J.
