Psychological adjustment to compliance, depression, and staff social support: their relation to objective compliance in ESRD patients

The authors hypothesize that compliance to treatment in ESRD patients depends on their psychological adjustment and social support. In 56 ESRD patients (31 HD, 25 PD), questionnaires concerning difficulties in following directives were filled out and compared to a staff support scale, where it is presumed that the poorly adjusted patient will be less compliant, and hence receive more staff support. Compliance was also related to somatic parameters like interdialytic weight gain.

The statistical analysis revealed that patient adjustment was well predicted by interpretation of the questionnaires, and that staff ratings for giving more support were related to negative responses towards regimen and depression. More staff support influenced patient survival via improved objective compliance. Differences between HD and PD patients appeared to be of no importance.

Comment: The results of this study suggest that there is a direct relationship between staff support and compliance, a fact that seems evident, but that is very hard to prove with objective data. (Johan Rosman M.D., The Hague, Netherlands)
