Left ventricular mass and diastolic function in primary aldosteronism and primary hypertension

The renin-angiotensin system may play a role in cardiac remodelling in hypertensive patients. AII can cause myocardial cell hypertrophy and hyperplasia, whereas aldo may cause interstitial fibrosis and collagen deposition. Patients with essential hypertension and primary aldo (n=34 ea) matched with respect to demographics and BP were compared using echo with respect to LV mass and diastolic function indices including flow during ventricular diastole (E) and atrial systole (A) . Patients with primary aldo had higher LV mass and reduced diastolic function (lower area E/A and higher A%). Excess aldo may result in increased LV mass and diastolic dysfunction. (D. Leehey)
