Similar effects of 3 months treatment with losartan and enalapril on albuminuria, blood glucose, and lipids in patients with essential hypertension

The effects of AII antagonists vs. ACE inhibitors on renal and metabolic parameters in essential hypertension (EH) have been presumed to be similar. However, there are relatively few studies addressing this point.

In a double-blind 12-week RCT, enalapril (E) 20 mg/d was compared to losartan (L) 50 mg/d in 93 patients with EH with normal renal function without overt proteinuria. Blood pressure and Ualb/creat ratios were reduced similarly in both groups. No differences in blood glucose or lipids were observed. In conclusion, at these doses, the drugs had similar effects on blood pressure, renal, and metabolic parameters.

Comment: This was a short-term study. It is unclear whether AII antagonists will have the same effects as ACE inhibitors in the long-term. The answer to this question requires long-term controlled studies. (David J. Leehey, M.D., Loyola University at Chicago)
