The role of homocysteine in the causation of atherosclerosis and thrombosis is increasingly being appreciated. Dialysis patients have quite high levels, and it is logical to attempt to normalize them in some way if possible.

Robinson measured plasma homocysteine levels in 176 ESRD patients and normal controls. Levels in dialysis patients averaged 26 uM vs. 10 uM in controls. Dialysis patients with levels in the upper 2 quintiles had a 2.9 relative risk of vascular disease.

Comment: Very promising is the possibility that, by giving industrial doses of folate to dialysis patients, elevated plasma homocysteine levels may be lowered into the near normal range. Also, when high flux dialyzers are used, plasma folate removal may be accelerated, requiring increased supplementation. (John T. Daugirdas, M.D., University of Illinois at Chicago)
