Renal disease in anti-phospholipid antibody syndrome (APLS)

Seshan et al reviewed the clinical course and renal biopsies of 26 patients with anti-phospholipid antibodies, excluding those with SLE. Sixty-five percent of patients had evidence of extra-renal thromboses, and the mean serum creatinine was 2.0 with mean urine protein of 4.4 gm/day with almost half of the patients being nephrotic.

Findings on renal biopsy included thrombotic microangiopathy, with or without "lupus like nephritis" usually consisting of membranous GN and nonspecific arteriosclerosis. Patients were treated with anticoagulation and/or immunosuppression. Follow-up based on treatment was not given; however, one patient developed SLE, three developed ESRD and 11 of 24 patients had improved renal function. Anticoagulation and/or immunosuppression have been used for treatment and while most of the patients did show improvement, it is unclear what role these forms of therapy played. (N.
