Simultaneous peritoneal catheter removal and insertion without interruption of PD

The authors report replacement of 34 PD catheters in 30 patients where catheters were replaced during the same procedure in which the previous catheter was removed using peritonoscopy. The replacement catheters were 2 cuff Swan neck Tenckhoff catheters. Thirty-one catheters were replaced for infection and 3 for dysfunction. PD fluid cell count had to be < 100. The procedure was performed under antibiotic (?) coverage which was continued for 2 weeks post catheter replacement. Patients were placed on nocturnal IPD (dry during the day) for 1 week.

Results were good, in that there were only 4 complications (2 leaks, 1 peritonitis, 1 scar abscess) .

Comment: Questions which arise on reading about his often discussed problem:
  1. What was the distribution of organisms causing peritonitis in these patients?
  2. How many peritonitis, exit-site infection, tunnel infections?
  3. How long were the patients treated with antibiotics before catheter replacement?
  4. Why were these (and not other) catheters removed?
  5. How long was the follow up after catheter removal?
