Menstruation, sexual activity, and contraception in women on dialysis

Holley et al administered a questionnaire regarding menstruation, sexual activity and contraception to 185 HD or PD women. The results obtained in 70 women aged < 55 y at the start of dialysis were analyzed. Mean age was 41 +/- 11 y, mean time on dialysis was 4.7 y. 70% have been pregnant but only 2% of pregnancies occurred while on dialysis. Menstruation was present in 64% and did not decrease after 4.7 y of dialysis, as it was for menstruation regularity. 19% had a hysterectomy and only 38% of those had a estrogen replacement therapy. 51% were sexually active (60% nonmenopausal, 20% postmenopausal, p=0.02). 38% of sexually active women used contraception. Only 14% of women reported to have discussed contraception with their nephrologist.

Comment: It is concluded that nephrologists should discuss menstruation and contraception with their patients. Post menopausal hormone replacement should also be addressed in these patients . (Denis Fouque, M.D., University of Lyons, France)
