Prognostic factors in anti-myeloperoxidase associated glomerulonephritis

Franssen et al tried to determine if outcome in 18 patients with anti-myeloperoxidase associated GN could be predicted based on persistent hematuria, nasal carriage S. Aureus, or cumulative prednisone/cyclophosphamide dose in the first year. None of these factors were predictive of patients who were likely to develop progressive renal insufficiency. However, progressive renal insufficiency was associated with detectable anti-MPO antibodies in 9 of 10 patients, while only 3 of 8 patients who did not have relapses or progressive renal insufficiency had detectable anti-MPO levels.

Comment: The authors conclude that persistently high anti-MPO levels are predictive of clinical course, however inadequate data are given to suggest the most effective form of therapy. (N. Kevin Krane, M.D., Tulane University)
