Better control of anemia by daily hemodialysis

Buoncristiani's "daily" home HD regimen, unlike that of Dr. Uldall (see Pierratos et al) is to treat for 1.5-2 hours daily
6-7 times per week. He presented data on 34 patients switched from
conventional HD to daily HD, focusing on changes in hematocrit and EPO dose.
The mean hematocrit increased from 25 to 30%, despite the fact that EPO dose
was reduced in 8 of the patients, some times (in 4 patients) to zero.
Comment: This data is in accord with that recently published in NEJM
by Eli Friedman's group in which Hct
improved with increasing Kt/V. There is the possibility of a small
confounder with daily home hemodialysis: because interdialytic weight gain
is less, one would expect less hemodilution and therefore a higher Hct in the
predialysis blood samples; however, a 5 point increase in Hct is greater than
would be expected from changes in hemodilution alone.
