Nutritional effect of 1.1% amino-acid containing dialysate in
Japanese malnourished CAPD patients: a multicenter study

This is the latest in a series of studies looking at new amino acid dialysate
solutions produced by
Baxter. It is a controlled study in a Japanese population of PD patients who
were malnourished
according to undefined criteria. Thirty-four of these patients received one
or two daily amino acid
exchanges for three months while a control group continued on conventional
treatment.
Five patients had side effects such as anorexia, nausea and vomiting,
presumably related to the
increased urea generation induced by the amino acid dialysate. Overall there
were modest but
significant improvements in the serum albumin in the treated group, and there
was also a significant
improvement in status by subjective assessment.
Comment: These findings are consistent with those of other studies
which show small and
sometimes significant improvements in indices of nutrition in response to
amino acid solutions. A
typical range of the usual side effects was seen. It is unclear whether the
benefits and the side
effects related to the number of amino acid exchanges used per day. A
reasonable conclusion at this
stage is that there are modest but significant benefits in some malnourished
patients using these
solutions. (Peter G. Blake, M.D.)
