Vancomycin-resistant enterococcal peritonitis

This abstract describes cases of peritonitis due to vancomycin resistant Enterococci (VRE). Six cases were observed over a 2-year period. Five of 6 patients were diabetic, and patients were on PD for an average of 35 months, All had exposure to vancomycin in the preceeding 6 months. The outcome: 4 out of 6 patients died within 6 weeks of the peritonitis episode. In 2 patients the infection was eradicated with alternative antibiotics.

Comment: This work underlines the seriousness of vancomycin resistance, especially in debilitated patients. New recommendations for peritonitis treatment by the Ad Hoc Advisory Committee for Treatment of Peritonitis are being developed in an effort to reduce vancomycin use in renal patients. Strict epidemiological measures must be enforced when vancomycin resistance appears in a renal unit. (Stephen I. Vas, M.D.)
