Treatment of CAPD-related peritonitis with ciprofloxacin:
results after 8 years

At this center ciprofloxacin was routinely used for initial treatment of
peritonitis. Treatment
schedule was 5 days of cipro 50 mg/l IP followed by ten days of cipro 500 mg
po TID. The study was
a comparison of the success rates of this therapy over time. Two time
periods were defined:
1988-1992 Success rate 83 %
1992-1995 Success rate 61 %
The conclusion of the study was, that ciprofloxacin therapy in the author's
particular geographic
region (bacterial resistance has geographical distribution) has become
inadequate due to emergence
of bacerial resistance.
Comment: Ciprofloxacin in my view never was an adequate first line
treatment of peritonitis.
Its role should be relegated to cases which cannot be treated with other
antibiotics (e.g.
hypersensitivity) and where the microbiological sensitivity results justify
its use.
