Acute treatment of constipation may lead to transmural migration of bacteria resulting in gram negative, polymicrobial, or fungal peritonitis
XVIth Annual CAPD Conference
Perit Dial Int Suppl 2 (Feb) 16:S45 1996

Five bedridden, hospitalized patients developed fecal peritonitis after treament for constipation. All five were hypoalbuminemic, and four had extensive vascular disease. All had received laxatives and 4 had been treated with enemas. Within 48 hours peritonitis supervened in each case. The outcome: 2 patients died, two were transferred to HD, and only one continued on PD.

Comment: This is a well known problem. There are no magic solutions except meticulous bowel movement control. (Stephen I. Vas, M.D.)
