Are internal jugular vein catheters suitable for daily home hemodialysis?

Tesio has long been using two separate silicone internal jugular catheters as opposed to a single double-lumen catheter for vascular access in his salvage patients. In this abstract he describes his results in 46 such patients. Technique survival was an amazing 98, 96, and 93% at 1,2, and 3 years, respectively. Mean blood flow rate was about 280 ml/min, and only 3 of these catheters needed to be replaced (2 for sepsis and 1 for clotting).

Comment: We don't use these catheters, but Dr. Tom Golper reports that he is using them with increasing frequency in his unit, and is quite satisfied with the result, although periodic urokinase infusion is required when the catheter flow becomes reduced. With such a high technique survival, the use of the Tesio catheter system becomes very attractive (theoretically) even for non-salvage patients wishing to follow a daily home hemodialysis schedule.
