The prevalences and clinical characteristics of ANCAs in
lupus nephritis

Chin et al from Seoul National University Hospital evaluated lupus
nephritis patients for ANCA, anti-MPO and anti-lactoferrin antibody in 59
patients (8 without nephritis). ANCA was positive in 37.3% of patients
with nephritis, most of whom had pANCA. With these patients having higher
anti-dsDNA antibody titers. This was associated with DPLN.
p-ANCA positivity was a marker for deterioration of renal function.
Patients with anti-lactoferrin antibody had lower serum creatinines.
Comment: This
study suggests that SLE patients with positive ANCA are more likely to have
nephritis and anti-dsDNA antibodies.
p-ANCA was associated with worse prognosis and DPLN. Anti-lactoferrin
antibody was associated with better renal function. This may help discern
what use ANCA plays in the evaluation of lupus nephritis.
