Long-term treatment with fish oil reduces renal progressionn in patients with IgA nephropathy (IgAN)

The authors summarize the long term (6.2 yrs) follow-up findings in the original cohort of patients with IgA nephropathy who were treated with fish oil (N Engl J Med 331:1194, 1994). Forty four of the 55 patients in the original group opted to continue taking fish oil after 2 yrs. The cumulative occurrence of renal failure defined as a serum creatinine of > 50% over the baseline at 5 yrs of followup was 21% and that of ESRD/death was 17% at 7 yrs compared to 40% at 4 yrs in the placebo group. As the authors point out, the value of these findings is limited by the absence of follow-up data in the placebo group followed for the same length of time. However, the data show that even in the treated group the outcome is not benign with nearly one in six patients dying or developing ESRD. (B.S. Kasinath, M.D., University of Texas at San Antonio)
