Fifteen year follow-up of reflux nephropathy (RN) by radiological imaging

This valuable patient set with reflux nephropathy and periodic follow-up has provided helpful prognostic information for the development of hypertension in this common pediatric renal lesion.

Intravenous urography (IVU) is reported 15 years after onset of reflux nephropathy in 37 patients with a median age of 28 years. Scar scores were recorded and compared with those from the original IVU films. I a third of subjects, intravenous urography performed at 15 yr follow-up demonstrated lesser degrees of scarring when compared with their original IVU, despite using the same scoring system. Whether this represents healing and renal remodeling or merely the inadequacy of our imaging techniques cannot be determined from this report. While it is recognized that renal scars may develop after surgical correction of reflux, the authors reasonably conclude there is no benefit to serial IVU many years later. A further caveat is made regarding the concordance of IVU and Tc99 DMSA scanning, although this has been well recognized. (Susan Mendley, M.D., Northwestern University, Chicago, IL)
