Glomerular tip lesion (GLT): clinical features, response to
corticosteroids and comparison to focal segmental glomerulosclerosis
(FSGS)

This is a timely abstract that deals with the important lesion of
glomerular tip lesion and asks
whether it is different from the focal sclerosis in terms of clinicla
features
and response to steroids. the strength of the abstract is the large number
of
patients with the GTL and their data is well defined with a decent follow-up
interval. The authors show that patients with GTL are more likely to be white
and
have a more pronounced nephrotic syndrome,i.e., more proteinuria, more
severe
hypoalbuminemia and hypercholesterolemia. Although the authors describe a
nearly
80% response to steroids compared to 33% of patients with focal sclerosis,
the
response was rather generously defined - aa reduction in proteinuria of 50%.
Considering that the mean proteinuria was 12.8 g/day a 50% reduction would
still
put them in the nephrotic range. It certainly appears that steroids do
reduce
the severity of nephrosis in these patients. However, the renal survival at

years was not different between the two lesions.
