Activity of lupus nephritis as related to serum levels of soluble Fas

Fas and Fas ligand levels were measured as markers of lupus nephritis activity. They were able to demonstrate that levels of sFas in 17 lupus patients with variable degrees of nephritis were significantly higher than in 17 controls. These levels were also significantly correlated to C3, soluble VCAM-1, soluble ICAM-1, and lupus disease activity index scores but not to C4 and anti-ds DNA antibody levels. Patients with WHO class III and IV were more likely to have positive sFas than patients with inactive lupus nephritis and that levels of sFas fell following steroid therapy which correlated with decreased inactivity index scores. The authors conclude that serum levels of sFas correlate highly with prevalence in activity of lupus nephritis and that serial measurements of these values may be useful in following activity.

Comment: Whether or not serum levels of sFas will be helpful in clinical management of lupus nephritis will require further study. (N. Kevin Krane, M.D., Tulane University School of Medicine)
