Lactic acidosis induced by stavudine (d4T)

Chronic lactic acidosis is a side effect of zidovudine (AZT)
inhibition of mitochondrial DNA polymerase. Mokrzycki and colleagues
report 3 cases of lactic acidosis induced by stavudine (d4T), a more
potent inhibitor of mitochondrial DNA polymerase than AZT.
Comment: These are the first documented cases of lactic
acidosis associated with d4T. Toxicity of nucleoside analogues should
be considered in the differential diagnosis of lactic acidosis in HIV
infected patients treated with these agents.
