Cardiac troponin I (CTI) is a reliable marker of acute myocardial infarction (MI) in end stage renal disease (ESRD) patients.

The use of troponins in diagnosis and prognosis of heart disease in CRF is limited by "false positives" in that population. This study compared Cardiac Troponin I with CPK-MB index in 6 HD patients with echocardiographically proven acute MI. The mean time interval to positive test, as well as the mean time to peak value, was the same for CTI and MB Index; neither test correlated with extent of injury as shown on echo. One patient had elevated CTI and normal MB Index as new injury shown on EKG and echo.

Comment: This study shows that two markers of muscle injury rise in tandem. Unlike other studies which attempted to establish the diagnosis of cardiac injury with troponins, this study starts out with patients proven to have acute MI by other means, i.e. the prior probability before doing the test is unity, so there is no issue of false positives. The precise role for troponin assays in ESRD patients remains to be determined. (James A. Sondheimer MD, Wayne State University, Detroit, MI)
