Transferrin (TF) and serum prealbumin (PAlb) are early
predictors of albumin (Alb) in chronic hemodialysis patients (CHD).

In this study, earlier markers of nutritional status were analyzed
prospectively in a group of 105
chronic hemodialysis patients. Using restricted residual maximum likelihood
based mixed effect
models, the intra correlation between transferrin, prealbumin, and albumin
were determined monthly
for 16 consecutive months.
The analysis revealed that early changes in both transferrin and serum
prealbumin significantly
predicted changes in serum albumin levels. The predictive power decreased as
the length of follow
up increased. A regression equation was developed which demonstrated that a
change of 0.1 g/dL in
albumin can be predicted by change of 8 mg/dL in the same direction by either
transferrin or serum
prealbumin levels. It was felt that both transferrin and prealbumin were
early and strong
predictors of serum albumin levels and that they potentially may be used to
identify patients at
risk for developing protein calorie malnutrition.
