Pharmacokinetics of quinupristin/dalfopristin (Syndercid RP
59500) in patients receiving continuous ambulatory peritoneal dialysis
(CAPD).

Quinupristin/dalfopristin is an antibiotic combination that may be
useful for treatment of vancomycin-resistant enterococci. Johnson and
colleagues measured the pharmacokinetics of the combination in CAPD
patients after a single intravenous dose. They found the kinetics of
the parent compounds and a metabolite unaltered in patients on CAPD
and that very little drug appeared in the peritoneal fluid.
Comment: It is useful to document the kinetics of the drug
during CAPD, but a more complete study would have included the
intraperitoneal administration of the compounds. From the current
study, the drug combination should not be used to treat ambulatory
peritonitis, because of the very low levels observed in the peritoneal
fluid.
