Can we reconcile metabolic acidosis and nutritional
status in hemodialysis patients?

Although acidosis has been shown to adversely affect muscle
protein metabolism, there is little clinical information
available about the inter-relations of acidosis and nutritional
indices in dialysis patients. Acchiardo et al. measured
predialysis serum HCO3, PCRn, Kt/V, dietary protein intake, serum
albumin and serum phosphorus in 131 dialysis patients. Patients
with low serum HCO3 levels (mean 18.4) had higher PCRn and DPI,
as well as higher serum phosphorus levels than those with higher
HCO3 levels. Serum albumin averaged 4.0, and weight and
hematocrit were stable in patients with "low" HCO3.
Comment: Mild degrees of metabolic acidosis are
not associated with adverse nutritional status.
