Effect of nondihydropyridine calcium antagonists on
progression of nephropathy from non-insulin dependent
diabetics: a five year follow-up study

The role of Ca antagonists on progression of nephropathy in patients with
NIDDM is
unknown. 52 patients (mean age 59) with NIDDM, albuminuria, hypertension,
and CRF
were randomized to recieve either lisinopril (n=18), verapamil or diltiazem
(n=18), or atenolol or guanfacine (n=16). BP goal was < 140/90. Loss of
Ccr
(ml/yr/1.73 m2) was -1.0, -1.4, and -3.3 in the 3 respective groups.
Proteinuria was decreased by 70%, 61%, and 11%, respectively. Renal death
occurred in 11%, 6%, and 38% of patients. Blood glucose and BP control were
similar in all groups. These data reveal a renoprotective effect of
nondihydropyridine Ca antagonists similar to that of ACE inhibitors in
patients
with NIDDM and nephropathy.
