Response to iron dextran therapy is not precluded by high serum ferritin

Serum ferritin is often elevated by factors independent of iron stores, leading to underdiagnosis of suboptimal iron stores in hemodialysis patients. In this study a course of IV iron was administered to HD patients with varying levels of serum ferritin. The primary finding was that a positive response to IV iron could be obtained in patients with TSAT less than 15% with serum ferritins as high as 600 ng/ml.

Comment: This confirms the poor sensitivity of the serum ferritin for defining inadequate iron stores in hemodialysis patients. An undefined issue would be the safety of administering IV iron to patients with relatively high serum ferritins given the link between cardiovascular risk and the serum ferritin concentration.
