The impact of plasmapheresis on recurrent focal
glomerular sclerosis in renal allografts

Recurrence of FGS in renal allografts may be due to a circulating humoral
factor that can be removed with plasmapheresis (PP). The effects of this
procedure were evaluated in a non-controlled fashion in 16 pts (17
allografts). Recurrence was diagnosed 3.1 +/- 5.7 mo after transplant.
13/17 patients had remission of proteinuria with the first course of PP
(mean number of treatments 7.5); 4/13 had a relapse of whom 2 responded to a
second course of PP. Mean length of remission was 24 mo in those responding
to the first PP course but only 0.9 mo in those requring a second course.
PP
appears to be beneficial in the majority of patients with recurrent FGS.
