Short-term comparison of ACE inhibition vs. calcium channel
blockade in African Americans with type II diabetic
nephropathy

In this small study 26 type II diabetic African Americans were
treated in a randomized protocol either with captopril 25 mg tid
or isradipine 5 mg bid. After 6 months the amount of proteinuria
had decreased in the converting enzyme inhibitor (CEI)-treated
group but actually increased in the group treated with calcium
channel blockers (CCB) despite a better control of blood pressure
in the CCB group. GFR as measured by iothalamate clearance did
not change in either group at 6 months time. The interim
conclusion is, that CEIs appear to reduce proteinuria in African
Americans with type II diabetic nephropathy.
