Acute renal failure following cardiac surgery

This was a prospective cohort study of 42,773 patients undergoing cardiac
surgery in 43 VA centers
between 1987 and 1994. A risk stratification algorithm was then validated in
an independent sample
of 3,795 patients undergoing cardiac surgery during 1994. The overall risk
of ARF requiring
dialysis was 1.1%. Independent risk factors for this complication were
valvular heart surgery (odds
ratio 2.0), baseline renal failure (1.8-5.8), preoperative IABP (3.2),
prior heart surgery (1.9), ejection fraction < 35% (1.5), rales (1.4), and
COPD (1.3). Advanced age
and diabetes were not risk factors.
