Patient survival on CAPD: comparison between Canada and the United States

This is perhaps the most important and disturbing abstract dealing with PD at ASN. It comes from the CANUSA database based on 680 patients in 14 centres in Canada and the USA. It shows that 2-year survival is 80% in Canada but only 63% in the USA> When a COX proportional hazards model is used and all risk factors including age, sex, racek comorbidity, serum albumin and dialysis dose and nutritional status are corrected for, thee is still an 87% greater risk of death in the USA compared with Canada. The authors suggest that this disturbing finding may be related to undetected differences in comorbidity or to compliance or to "quality assurance policies". The higher mortality in the US cf Canada, could be related to the fact that candidates for dialysis treatments are much more carefully screened in Canada, and the US has greater numbers of poverty stricken, undernourished, elderly people receiving dialysis.
