Bone mineral loss in chronic renal failure: a difference
between young and old CRF patients

Bone mineral density of the lumbar spine and femoral neck measured with dual
energy X-ray
absorptiometry was measured in 85 patients with CRF and mean GFR 23 ml/min.
Both were
lower than normals (apparently not statistically significant). There were no
differences in
gender. The bone density of the femoral neck was lower than that of the
spine and correlated
positively with the calcitriol and negatively with the PTH levels. In
comparing patient >60 yrs
versus those younger, the spine density was more decreased in the younger
group. In older
patients; femoral neck density correlated with calcitriol levels but in the
younger, the density
correlated negatively with PTH levels. The authors conclude that in older
patients, bone loss is
related to decreased calcitriol synthesis whereas in the younger, to
increased PTH secretion.
Comment: Other variables, such as GFR, ovarian function, alchohol
ingestion, and treatment
regimen need to be assessed.
