Hypertension, cardiomyopathy, cardiac morbidity and mortality in ESRD

Yearly echocardiographic follow-up of 432 dialysis patients permitted analysis of the relationship between mean arterial pressure and cardiac structural disease. Each 10 mm Hg rise in MAP was associated with concentric LV hypertrophy and increase in LV mass index and cavity volume on follow-up echo. Increased MAP also was associated with de novo cardiac failure and ischemic heart disease. Low MAPs were associated with death following admission for cardiac failure. Thus, the results suggest that the mortality risk of low MAP in dialysis patients is related to cardiac failure. The effects of moderate increases in MAP on mortality are difficult to demonstrate, but this study finds the well-established link between hypertension and cardiac disease in this patient population.
