Peritonitis influences mortality in PD patients

Mortality in the population of one large PD unit was studied, summarizing
results over a period of 5
years. The group comprised 516 patients (mean age 50 years) with a total of
896 years on CAPD
(mean: 2.1 years), with an average peritonitis rate of 0.8 episodes/year.
Survival was 82% at 1 year, 72% at 2 years, and 41% at 5 years. An increase
in the peritonitis rate
was a risk factor for whites, for non-diabetics, and for elderly patients.
Although introduction of
the Y set improved peritonitis rates, the survival rates did not improve.
This finding was
explained by only one death due to S. epidermidis peritonitis (0.5% of S. epi
peritonitis) versus 13
deaths due to Gram negative and/or fungal peritonitis (25% of Gram negative
and/or fungal
peritonitis).
Comment: To reduce mortality, improvements must be made in reducing
the incidence of and
optimizing therapy of peritonitis due to Gram negative and fungal
peritonitis.
