Increased homocysteine values, a risk factor for premature
coronary artery disease, prevail in patients on hemodialysis

Elevated plasma homocysteine (H) levels are associated with
premature vascular occlusive disease. In dialysis patients high
H levels are also seen, more so in patients with folate
deficiency. In this study H levels were measured in 133 HD
patients along with levels of folate, B12, and pyridoxal,
5'phosphate. The H levels averaged 36 umol/L in HD patients vs.
12 umol/L in normal controls. There was a strong inverse
correlation in HD patients between serum folate levels and plasma
H levels. However, serum folate levels in HD patients were not
reduced overall.
Comment: The speculation is,
that high plasma H levels in HD patients may contribute to
vascular disease. The data suggest that increased attention
should be paid to folate supplementation in this population.
