Cytokine removal and cardiovascular hemodynamics in patiens with ARF and treatment with CVVH

This is a prospective study of cytokine removal by CRRT. CVVH was used in 33 patients (18 with septis and 15 with primarily cardiovascular problems) with ARF. Multiple cytokines, including TNFà, IL-6 and IL-1 and their receptors were measured in the blood and ultrafiltrate at different time points after initiation. No significant changes in blood levels of cytokines were seen, although TNFà was detected in UF. Hemodynamic improvement was seen in 24 hours, but could not be ascribed to cytokine removal. This study is similar to the ASN95 abstract by Manns et al. which also found hemodynamic improvement in sepsis with CRRT, but the data presented here do not demonstrate that this improvement is associated with cytokine removal. Unfortunately the serum levels of cytokines are misleading and do not correlate en linear fashion with clinical status.This is largely secondary to the well konwn paracrine effect on target organs (Vega)
