Long-term effect of captopril on kidney function in normotensive insulin-dependent diabetic patients with diabetic nephropathy

32 normotensive patients with IDDM and nephropathy were randomized to Rx with captopril (12.5-125 mg/d) or to a control group and followed for 8 years. BP was constant in the treated group (128/78 - 129/77 mm Hg) but increased in controls (127/79 - 137/84). Fractional albumin clearance was unchanged in the captopril group but increased in controls. The GFR decline was similar in the two groups. Captopril has a long-lasting beneficial effect on BP and proteinuria; however loss of GFR is minimal providing normotension is sustained. (Leehey)
