Non-suppressible parathyroid hormone secretion is related to
parathyroid gland size in uremic secondary hyperparathyroidism

The relative importance of parathyroid gland enlargement and insensitivity to
the suppressant
effects of Ca in the development of uremic hyperparathyroidism is unclear.
The authors studied
the relationship between PTH suppression in response to increased Ca and
measured gland size
in 19 patients on chronic hemodialysis. The slope was abnormal but the set-
point was normal.
Non-responders (undefined criteria) in % PTH suppression had a lower slope
and greater gland
size compare to responders. The variable degree of calcium non-suppression
of PTH secretion is
likely due to parathyroid gland hyperplasia.
