Magnitude of end-stage renal disease in IDDM: 35 year
follow-up study

This study examined the risk of ESRD in 141 white Massachusetts residents
who first came to Joslin Diabetes Center in 1959 with newly diagnosed IDDM.
The Center emphasizes tight blood sugar control. They ascertained during the
following 35 years the occurrance of ESRD in this cohort. The first case of
ESRD occurred after 14 years of diabetes, and the incidence rate increased
to
a peak in the interval 25-29 years after diabetes. After 35 years of IDDM,
the
cumulative incidence of ESRD in the cohort was 24%. The renal replacement
therapy extended the life of ESRD patients by 2.5 years. The study concludes
that despite an emphasis on tight blood sugar control and the introduction
of
new modalities for the treatment of diabetic nephropathy, one out of four
patients
with IDDM developed ESRD.
