Kidney/pancreas transplant versus kidney transplant alone

The relative benefits of kidney-pancreas (KP) vs. kidney transplant alone
(KTA) are unclear. Outcomes of cadaveric KTA vs. KP in comparable patients
groups
at U Minnesota were studied and compared to outcome in patients receiving
kidneys
from living-related donors (LRD). Infectious death was more common in KP
(14/54 patients developed peripancreatic infection requiring pancreatectomy,
of
whom 6 died within 12 mo); other causes of death occurred with equal
frequencies among the groups. 3-yr survival was 86% in LRD, 90% in KTA vs.
68% in KP (p < 0.01). A randomized trial of KP transplant is needed to
determine if the long-term benefits are worth the increased short-term
risks.
