The efficacy of ACE inhibition in preventing diabetic nephropathy
(albuminuria > 300 mg/24h) was investigated in a prospective randomized
study
of 44 normotensive patients with IDDM and persistent microalbuminuria
(albumin
excretion rate 30-300 mg/24h). 21 patients recieved captopril + thiazide; 23
patients
were untreated. BP, albumin excretion rate, and GFR were followed for 8
yrs.
40% of controls and 10% of treated patients developed nephropathy. ACE
inhibitors postpone the development of diabetic nephropathy in normotensive
patients with IDDM and persistent microalbuminuria.
