Renal gallium scintigraphy helps decision-making in acute
renal failure

The usefulness of renal gallium scintigraphy to diagnose drug-
induced or other acute interstitial nephritis (AIN) is not very
clear. In this study gallium uptake was measured in 66 patients
with acute renal failure of intrinsic nature. 32 patients had
significant uptake bilaterally and biopsy in 14 of these was
consistent with AIN. 6 patients had unilateral uptake only,
suggesting acute pyelonephritis on one side. The remainder had
no uptake and most of them were thought to have ATN.
Comment: The conclusions made are that gallium
scintigraphy is helpful in making a diagnosis between ATN and
AIN. However, many of the patients were not biopsied and the
renal diagnosis remained presumptive.
