Cyclosporine nephrotoxicity in insulin-dependent diabetic patients: an eight year follow up

Cyclosporine has been used to treat patients with early insulin dependent diabetes mellitus in an attempt to abort the disease. In this randomized study, 21 patients received cyclosporine for 12 months and 19 patients received placebo. Over an 8-year follow-up 4/18 cyclosporine-treated patients developed persistently elevated urinary albumin excretion, whereas UAE remained normal in the placebo-treated patients, with the exception of 1 patient with biopsy-proven IgA nephropathy. A kidney biopsy in 5 of the cyclosporine-treated patients was consistent with cyclosporine-induced nephropathy. Furthermore, the GFR declined in the cyclosporine-treated group, whereas it rose in the placebo-treated group. The results suggest that cyclosporine treatment of young diabetic patients may not be benign and may accelerate the progression of renal disease. (Daugirdas)
