Effect of aging on renal responsiveness to parathyroid
hormone in healthy men

It is unclear if the impaired production of calcitriol in response to PTH
infusions in the elderly
is
due to aging or reduced GFR. 16 healthy and 9 elderly (mean age 70 yrs)
subjects with GFR>70 ml/min
underwent a 24 hr PTH infusion. At baseline there were no differences in
calcitriol levels but the
PTH levels were 3-fold higher in the elderly. There were no significant
differences in the rise in
calcitriol levels or in the urinary cAMP and FEPi in the 2 groups. The
authors suggest that the
blunted response to PTH reported with aging reflects an age-related decrease
in GFR.
Comment: It would be interesting to know the authors' explanation for
the same levels of
calcitriol at baseline despite the higher PTH levels in the elderly.
