Urinary C5b-9 in early and advanced renal insufficiency in
membranous nephropathy

The urinary excretion of the terminal component of complement
C5b-9 was studied in 20 patients with idiopathic membranous
nephropathy (MN) and 2 patients with lupus MN. Urinary excretion
of C5b-9 was correlated with the degree of proteinuria and
progression. In patients excreting an average of 8 g/day of
protein, 40% of urines were positive for C5b-9, whereas in
patients excreting an average of 2.2 g/day of protein, only 22%
of samples were positive for C5b-9. C5b-9 was also found more
frequently in those patients in whom renal function as measured
by ClCr was deteriorating. C5b-9 is proposed as one additional
marker to follow the course of MN.
