Effect of insulin-like growth factor 1 on nitrogen balance in malnourished CAPD patients

This abstract describes continuing work from Kopple et al on the use of recombinant IGF-1 in malnourished CAPD patients. It describes 6 patients who spent 35 days in a CRC. This comprised 15 days for baseline observations and 20 days of rhIGF-1 therapy at 50 or 100 #g/kg twice daily. The authors find dramatic drops in BUN and dialysate N losses, with a markedly positive N balance on rhIGF treatment. Serum albumin and anthropometric indices did not change. The authors conclude that the anabolic effect is significant and mainly involves somatic protein.

While these observations are interesting, there is clearly a need to prove the viability and safety of this therapy over longer periods of time in dialysis patients. (Blake)
